This Privacy Policy is not a contract and does not create any contractual rights or obligations. We will update this Privacy Policy from time to time to reflect our practices and recommend that you periodically review this Privacy Policy.

If we make material changes to this Privacy Policy, we will make reasonable attempts to notify you before they become effective. If you have questions about this Privacy Policy, please feel free to contact us (our contact information is at the end of this Privacy Policy).

Important Disclaimer: This website (fabrichealth.com) is provided for informational and business purposes only. It is not intended for patient care or the submission of Protected Health Information (“PHI”). Do not use web forms or other features of this site to submit medical, diagnostic, or treatment information. Any such information inadvertently received will be promptly deleted consistent with our data handling protocols.

A Note About Protected Health Information

The Solution supports healthcare organizations and patients with various objectives, including appointment scheduling, care navigation, among others.

If you use the Solution as a patient of a healthcare organization, we will collect certain information about you on behalf of your healthcare organization that may be Protected Health Information regulated by HIPAA. Where this is the case, we act as a “business associate” to your healthcare organization, and your healthcare organization’s Notice of Privacy Practices and our agreements with your healthcare organization govern the use of your Protected Health Information, not this Privacy Policy. If you have any questions about the use and disclosure of your Protected Health Information, please contact your healthcare organization.

For clarity, PHI should only be submitted and processed through designated patient portals or clinical platforms, not through the public-facing fabrichealth.com website.

We collect information you provide to us through the Services, for example when you access our website, create or modify your account, register to use our Services, schedule appointments, make payment for healthcare services, request information from us, contact customer support, fill out any form in the Services, or otherwise communicate with us.   • Payment information (credit card or debit card number, expiration date and credit card security code)

  • Demographic information

Do not provide health or medical information, including PHI, through the Site’s general contact forms. Submissions of PHI through these channels will be deleted.

Evo, by Fabric®, is not health insurance and is not intended to replace comprehensive health coverage. Services are not a substitute for emergency care or health plan benefits. Visit fees vary depending on service type and may not be covered by insurance. Telehealth visits may not be appropriate for all conditions and are not a substitute for in-person care in all cases.
